Health Insurance Enrollment

Our goal is to provide safe, secure and stable housing support for individuals in need. We’ll help connect you to housing resources while coordinating with your healthcare providers to ensure all your needs are met. Those who are eligible for supportive housing include, HIV-positive single men and women, people of transgender experience, HIV-positive women recently released from a correction setting, HIV-negative individuals with other chronic health conditions, and HIV-positive LGBTQ youth (ages 18 to 24).


Pharmacy

Housing Works Community Healthcare can help you access affordable medications and fill all your prescriptions at locations that are conveniently co-located at each of our sites throughout our partner pharmacies – so you’ll never have to go out of your way to get what you need. We can help connect you to housing resources while coordinating with your healthcare providers to ensure your needs are always met. Housing is available to eligible HIV-Positive men and women, transgender individuals, and formerly incarcerated members of the community.
