Controlling Hypertension in East Harlem with CHCANYS
Over the last year, our 125th Street Community Health Center partnered with CHCANYS (Community Health Center Association of New York State) to improve hypertension control across East and Central Harlem. As a member of the collaborative, Housing Works partnered with neighboring healthcare and program providers to implement and evaluate strategies to prevent and manage cardiovascular disease.
We do healthcare differently. Housing Works strives to provide community-first care, with a focus on harm reduction and services that are affordable with or without insurance. Whether you need quick access to PrEP, a gender-affirming therapist, or a primary care doctor who won’t judge your experience–we’ve got you. Office visits and telehealth appointments available.
