We are excited to announce that all our health centers were recognized by the National Committee for Quality Assurance as a patient centered medical home (PCMH)! PCMH highlights clinicians and practices who demonstrate they provide high-quality patient-centered primary care in the following areas:
- Team-based care and practice organization
- Knowing and managing your patients
- Patient-centered access and continuity
- Care management and support
- Care coordination and care transitions
- Performance measurement and quality improvement
Our Westside, Downtown Brooklyn, East New York, and 125th Street health centers also received Distinction in Behavioral Health Integration. This highlights practices that put resources, protocols, tools and quality measures in place to support the broad needs of patients with behavioral health related conditions.
These recognitions demonstrate our commitment to continuous quality improvement and a patient-centered approach to care. Our patients can be confident that they receive care that has been vetted based on industry-leading best practices.
