Coming Soon: 125th Street Health Center

Opening Summer 2024!

Our new 125th Street Health Center will serve the East Harlem community with comprehensive, integrated care. Our 125th Street location will offer a range of services including primary care, psychiatry, and care coordination. We recently hosted a ribbon cutting where we received a generous donation from Anthem Blue Cross Blue Shield to support our clients through a new virtual food bank program in collaboration with EngageWell IPA. More details to come about when we will officially open for care!
